As you may know, there are new federal regulations that protect the confidentiality of your individual health information. As an applicant to The Genentech Endowment for Cystic Fibrosis you and your physician have provided certain information in order to apply for the program.

To help administer The Genentech Endowment for Cystic Fibrosis, the Endowment's Board of Directors have contracted with a third party, Lash Group Healthcare Consultants, located in Charlotte, North Carolina. In this role, Lash Group has received certain health information about you and may be required to obtain additional information.

Maintaining your privacy is important to The Genentech Endowment for Cystic Fibrosis and to Lash Group. In connection with the receipt of your health information, The Genentech Endowment for Cystic Fibrosis and Lash Group are required by law to protect the privacy and confidentiality of your individual health information. The Notice of Privacy Practices describes how your health information may be used or disclosed and how you may obtain access to this information.
